Constellation Health Centre
The Centre de santé Constellation Health Centre is a bilingual primary health care clinic in Whitehorse that opened on November 7, 2022. It is the first primary health care centre of its kind in the Yukon.
Constellation Health Centre connects clients to quality health care when they need it, delivered by a team of interdisciplinary health professionals. A range of everyday health and wellness services are offered under one roof in both French and English, with virtual interpretation also available in other languages.
Clients will be connected to primary care providers, nurse practitioners or physicians, who will:
- liaise with the rest of the clinic’s primary health care team;
- connect clients to any additional health services they may need; and
- coordinate clients’ care.
This means clients will receive the right care by the right provider at the right time.

About primary health care clinics
A primary health care clinic is where people go for integrated health and wellness services. Primary health care providers liaise with other providers and specialists across the health and social system to coordinate people’s health and wellness care.
What do primary health care providers do?
Primary health care providers are licensed medical professionals who have advanced education and clinical training. They’re the first point of contact when clients need non-urgent care or advice about their health and wellness.
Primary health care providers:
- provide preventative care;
- identify and treat common medical conditions;
- order and interpret diagnostic tests;
- prescribe and monitor medications; and
- make referrals to specialized providers.
How does the centre work?
Once accepted to the Constellation Health Centre, clients will have access to an interdisciplinary team of providers including physicians, nurse practitioners, a registered nurse, a licensed practical nurse, and social worker. This team of providers will work with the client to determine their care goals and ensure clients are connected with the services and supports required to achieve their health and wellness goals.

Why is the Government of Yukon creating the Constellation Health Centre?
This new bilingual health centre is a commitment by the government to provide improved access to care for all Yukoners. It adds an access point to primary health care services for Yukoners in French and English and will help reduce pressure on our health care system.
The Constellation Health Centre is the first of its kind in the territory. It acts as a catalyst to begin building team-based, integrated, primary health clinics throughout the Yukon.
What services does the new Constellation Health Centre offer
You’ll have access to a range of everyday primary health and wellness services. Services are offered under one roof in both French and English. This includes:
- outpatient care;
- referrals to other health care providers and specialists;
- diagnosing, ordering and interpreting diagnostic tests, prescribing and/or administering pharmaceuticals and prescribing and/or performing specific procedures;
- virtual visits;
- routine check-ups;
- chronic disease management;
- health education and promotion;
- social work and rapid-access mental health counselling.
Will the centre be a walk-in clinic?
No. In contrast to a walk-in clinic, clients will be registered with the clinic. You’ll have access to a team of care providers, including primary care providers such as nurse practitioners. They’ll provide you with primary health care services and connect you to additional health and wellness services depending on your needs. All care will be provided at the clinic in Whitehorse, with some virtual options available.
The Constellation Health Centre offers same-day appointments. This means you’ll be able to call the centre and pre-book an appointment, provided there is an opening, to receive care on the same day. Same-day services mitigate long wait times often associated with walk-in clinics. They also take stress off the hospital and other primary care clinics.
Is the bilingual health centre only for Francophones?
No, the centre provides services in both French and English, and all Yukoners can apply to become a client. Most of the staff speak French and virtual interpretation is also available in other languages.
