The Centre de Santé Constellation Health Centre has moved from its temporary location to its permanent space at 9010 Quartz Road in Whitehorse. The new space will enhance the clinic’s ability to offer high-quality, primary health care to Yukoners in both English and French.
The team of interdisciplinary health professionals at Centre de Santé Constellation Health Centre currently serves 816 Yukoners and has been open since November 1, 2022.
Clients of the clinic will have access to an interdisciplinary team of providers including nurse practitioners, a registered nurse, a licensed practical nurse, and a social worker. This team of providers will work with clients to determine their care goals and ensure clients are connected with the services and supports required to achieve their health and wellness goals.
Applications continue to be prioritized and accepted. Yukoners not initially accepted will be added to the waitlist and notified when space becomes available.
Yukoners over 16 years of age with a valid Yukon Health Care Insurance Plan number are eligible to become clients of the clinic. Individuals younger than 16 years of age may have a guardian apply on their behalf. Yukoners on the Find a Primary Care Provider waitlist who are interested in receiving care at the Constellation Health Centre must also apply.
The new permanent home for the Centre de Santé Constellation Health Centre represents an important step forward in building a strong, resilient and responsive healthcare system for all Yukoners. The multidisciplinary hub, which offers access to a range of services in both French and English, is the first of its kind in the Yukon. Since opening, the facility has helped strengthen the Yukon’s health care, in particular for the territory’s francophone population. We look forward to its continued positive impact on our community.
The Centre de Santé Constellation Health Center is a remarkable success story driven by our commitment to improving access to government services in French. We strive to support our growing Francophone community and we are working hard to ensure all Yukoners have access to the health care services they need. I am grateful to our community partners, l’Association franco-yukonnaise and the Partenariat Communauté en Santé, for their dedication to this project and to the wellbeing of the Yukon French-speaking population.
We are really pleased that the Health Centre has moved to its permanent location as it will increase the number of Yukoners served. We are hoping that bilingual doctors will join the Constellation Health Centre in the very near future. We will continue to collaborate with the Centre in recruiting more bilingual health professionals to provide French services.

Health care is offered in both French and English, with virtual interpretation also available in other languages.

The interim space at 102-4149 4th Avenue is now closed and services are being offered from the permanent location at 9010 Quartz Road, beside the Whitehorse Health Centre.

Opening hours are Monday to Friday from 9 am to 6 pm (closed on statutory holidays).
