Register for pulmonary rehabilitation for chronic conditions

You can register for pulmonary rehabilitation. This program will help you manage Chronic Obstructive Pulmonary Disease (COPD). It will also help you manage other lung conditions such as:

  • bronchitis;
  • emphysema;
  • asthma; and
  • pulmonary fibrosis.

This program includes:

  • 1-on-1 assessment with a registered nurse and clinical exercise specialist;
  • support of a certified respiratory educator (CRE);
  • group education sessions that help you learn how to track and manage your condition;
  • information on local support resources; and
  • group supervised exercise programs that help you to build and maintain:
    • physical fitness; and
    • long-term independence.

Steps for registering for pulmonary rehabilitation

  1. Go to your family clinic or any health care provider.
  2. Get a referral from the health care provider.
  3. Call or visit CCSP office.
  4. Attend an intake appointment at the CCSP office and discuss your health goals with the team.
  5. You and the CCSP team will discuss and decide on when you will start the pulmonary rehabilitation program.

You do not need to be referred by a health care provider, you can skip to step 3 if you want to self-refer.

If you self-refer, we will inform your health care provider.
